Sculptra side effects: why do Sculptra nodules form, and what should I do if they appear after I return home?

Short answer

Depends on the case

Most Sculptra lumps (nodules) are not visible, can only be felt, and decrease over time. The main cause is too much product collecting in one place, so they can be greatly reduced by adequately diluting the product and distributing small amounts broadly in the deep subcutaneous layer. Lumps can appear from a few days after treatment to as late as 14 months after the last session, so if you feel one after returning home, send us photos first. We will assess the photos and tell you what to do next.

Why do Sculptra nodules (lumps) form? Are they a failure, and how can they be reduced?

Feeling a lump does not mean the treatment has failed; most can only be felt and decrease over time. The main cause of Sculptra nodules (lumps) is too much product collecting in one place, and I see this not as a matter of chance but as a problem that can be almost entirely addressed by the quality of the injection technique. Nodule formation is influenced less by the number of vials used than by the injection depth and how evenly the product is distributed, which is why depth and distribution are what I focus on most to reduce nodules.

At our clinic, we adequately dilute each vial to 8–10 mL and usually use a cannula to distribute small amounts broadly in the deep subcutaneous layer. The U.S. instructions for use also advise against injecting too superficially, and the thinner the skin, the more attention depth deserves.

Immediately after treatment, we massage the injected areas evenly. At home, we instruct you to massage for 5 days, 5 times a day, for 5 minutes each time. Follow this aftercare, but the first requirement is to inject at the proper depth and prevent the product from collecting in one place from the outset.

How often do Sculptra lumps occur, and when do they appear?

Most Sculptra nodules are not visible and can only be felt. In one retrospective study of 130 patients, nodules were reported in 11 patients (8.5%), and all were found within the first 4 months. However, treatment area, depth, dilution, and distribution technique all differ, so the figure from this one study should not be treated as the rate for every Sculptra treatment performed today.

The timing varies. The U.S. instructions for use state that papules and nodules can develop from days to months after treatment, with cases reported from 1–2 months to 14 months after the last treatment. They usually occur at an injection site, are painless, and are not visible.

Fullness that you can feel immediately after treatment is not automatically considered a nodule. Swelling mixed with the diluent can make the area feel temporarily uneven. Common reactions such as swelling, bruising, and soreness usually improve within about 1–3 days, and How should I massage after Sculptra, how many days does swelling last, and can I fly the next day? explains the recovery period and massage at home. If a firm pellet remains in one spot after the swelling goes down, or a new lump develops months later, that is when we assess it separately as a possible nodule.

What should I do first if I feel a Sculptra lump after returning home?

If you feel a Sculptra lump after returning home, send us photos first so we can assess it. A lump that can only be felt, is not painful, and is not getting larger will usually decrease over time. The U.S. instructions for use also state that Sculptra nodules are usually palpable and asymptomatic, and that they can take some time to resolve.

If fewer than 5 days have passed since treatment, continue massaging as instructed: 5 times a day, for 5 minutes each time. If the lump gets larger, painful, or red, do not keep watching it; send us photos right away. There is no single treatment for every lump, so we will assess its condition and guide you on what care to seek at a local clinic.

In case you need to visit a local clinic, keep your own record of the date and area of the Sculptra treatment, the date when you first noticed the lump, and photos showing how its size changed afterward. The local medical team can make a faster assessment if they can immediately identify what was injected and when. In the rare case of a change in vision, severe pain, or skin turning white, do not wait for a reply to your photos: seek medical care right away where you are.
